Sign-ups are held between the hours of 8:30 am -12:00 pm, and 1:00 pm – 4:00 pm, on the specified dates.
IN ORDER TO APPLY:
*Social Security numbers must be provided for all household members.
*You must know your annual income amount.
*Proof of residency in Chautauqua County must be provided.
