Alzheimer’s Caregiver Support Services
The “Caregiver Support Initiative for Underserved Communities” focuses on reaching out to individuals who are the primary caregivers for someone with Alzheimer’s Disease or a related dementia.
|Outreach – Targets underserved individuals, rural areas and the Hispanic population throughout the area.|
|Education and Support – Conducts education programs which include a variety of topics for caregivers of the community.||

|Support Group – Groups meet monthly. Support Groups provide opportunities to share experiences, offer support, ideas, and resources.|
|Assessments – Individual caregiver assessments are performed by an RN and education and resources are available to the caregiver.|
|Respite Care – No Cost Respite services are available to provide relief to caregivers by providing a temporary reprieve from daily responsibilities.|
