SERVICE AREA: Luke's Place -
A Transitional Independent Living Program (TILP)
Description of Program / Service Provided:
The Luke's Place program is designed to provide a
semi-independent living option for youth ages 16-21 who are homeless, in
danger of becoming homeless, or live in an unsafe environment.
The program has two fully furnished apartments.
Each has occupancy for two program participants of the same
gender.
While in the program, a needs assessment is completed with
the youth and appropriate services are provided based on the assessment.
Available services include:
-budgeting
-self
sufficiency facilitation services
-counseling
-referrals
-advocacy
-life skills
education
-staff
available 24 hours a day
History:
Luke’s Place apartments were established as an
extension of the Safe House of Chautauqua County.
The program was created by a collaborative funding effort
of St. Luke’s Episcopal Church, New York State Homeless Housing Assistance
Program, City of Jamestown, Sheldon Foundation and the Hartley Fund.
