An extension of the MOMS Prenatal Support Services
Program, the Prenatal Home Visiting Program is designed to help pregnant and
post-partum women achieve the best possible birth outcomes for their babies.
The focus of the program is on individualized instruction
to help strengthen the mothers’ knowledge and involvement in their health
care, both during and after their pregnancy.
For those that qualify, home based assessments, visits,
and support contacts are conducted to address prenatal and post-partum needs
throughout the pregnancy and up to 12 weeks post-partum. These are provided
at the convenience of the mother, usually on a monthly bassis and/or based
upon her needs.
Services include health and nutrition assessments and
referrals as well as advocacy and referral to other services as needed.
All services are confidential and are offered throughout
Chautauqua and neighboring Counties.
Program is linked to the WIC Supplemental Nutrition
Program for Women, infants, and Children, the Breast Pump Rental Station and
the Breastfeeding Peer Counselor Programs, and the Family/Child Health Plus
Facilitated Enrollment Insurance Program.
