County to residents who may be recovering from an illness, be chronically
ill or disabled, or just need assistance with the basic activities of daily
living.
Provides housekeeping, personal care, home health aide, and skilled
nursing services.
Provides services 24 hours a day, 7 days a week.
A personal plan of care is established in cooperation with your family
and under the direction of your physician.
A Registered Nurse will make an initial free assessment within 48
hours of your call for service.
Employs over 120 field staff whose references and background are screened
before employment is established.
Licensed by the New York State Department of Health since 1986, and
authorized to provide services throughout Chautauqua County.
Contracts with other local agencies such as the Office for the Aging,
CASA, Lake Shore Long Term Program, the Developmental Disabilities Services
Office, Hospice, the Veteran’s Administration and private pay.
