PROGRAM: Child Health Plus & Family Health Plus Facilitated Enrollment 

Description of Program / Service Provided:

Facilitated Enrollment provides health care coverage for pregnant women, children under nineteen years of age and adults 19-64 years old.

The coverage is either free or low-cost, depending on income eligibility.

Enrollers provide assistance in the application process and check an applicant’s eligibility for health care coverage such as Child Health Plus, Family Health Plus, Medicaid or any Medicaid Managed Care Plan options.

The Enroller also handles the application, follows up on its status, and makes sure the applicant receives the coverage they are eligible for.
