TERMS OF AGREEMENT
1.
The
person or persons who make the reservations for catered functions with COI
should do the following:
A.
Two
weeks to one month prior to the event
-
Select
a menu from our catering choices or arrange for custom menu with Food
Service Manager.

Fill
out Food Service Special Event Form. Provide the following
information: Group name or program name, contact name, phone number and
billing information.

Indicate
anticipated number of guests, date, time, and location.

All
COI orders must be accompanied with a purchase order.
-
When
completed, please send a copy to Food Service Manager and either Head Cook
at Connections North and/or Connections South.
B.
Eight days prior to the event:
-
Submit
a guaranteed number of guests.
-
COI
will prepare food and settings 5% above the guaranteed number of guests. The
customer will be billed for actual number of guests or guaranteed number
whichever is greater.

Full
payment by cash or check is due the day of function unless special credit
arrangements have been made.

ANY
ORDERS SUBMITTED IN LESS THAN 8 DAYS ARE SUBJECTED TO MARKET PRICE AND
MENU CHOICE.
2.
Evening
functions will incur a surcharge of 15%.
3.
Additional
information:
-
All
prices are subject to a 10% - in house or 15% - off premises service charge
that requires
setup.
-
Delivery
charges are applied to locations other than COI facilities.
Anything over five miles will be charged the going mileage rate.

All
prices are subject to change.
We
would be glad to design a customized menu for you.
