DEPARTMENT: Health Services

PROGRAM: Minority Outreach for the Developmentally Disabled

Description of Program / Service Provided:

Provides direct advocacy to minorities with developmental disabilities, including their family members.

A developmental disability would include cerebral palsy, mental retardation, autism, neurological impairment, or other related conditions that began before 22 years of age.

Accessing services such as transportation, translation, training, support, treatment, respite by linkages and referrals to other agencies.

Families are empowered to access health services through advocacy and networking.

Offer a full-time outreach specialist in the North County and part-time in the South County, who are specially trained to work with the developmentally disabled. Education is updated regularly.

Providing education to access financial resources and maintain records is also part of the program goal.

Services are free of charge.

 

History:

Funded through the Western New York Developmentally Disabled Service Office and contracted to COI due to our experience in reaching the minority population.

Eligibility:

Certain Developmentally Disabled individuals who are diagnosed before age 21.
