PROGRAM: Community Health Worker Program

Program Overview

• Designed to help prenatal women achieve the best possible birth outcomes for themselves and their families

• Enrollment in continuous and comprehensive prenatal services

• Assistance in overcoming barriers to access service

• Assistance with health insurance enrollment

• Assessments with action plan for identified needs

Service Provided

• Services are provided through home visiting with a Community Health Worker who will assist with health and social needs

• Screenings and Health education provided in home setting

• Assistance with linkages to support services

• Prenatal and post-partum case management services
